Pre-Appointment Checklist

  1. Completed patient information forms

  2. Insurance card(s)

  3. Driver’s license

  4. Any previous medical records, especially test results

  5. Please bring your glasses, computer glasses, sunglasses, and contact lenses

You will receive a link via text/email prior to your appointment to electronically fill out paperwork.

Appointment Cancellation Policy

Please provide at least 24-hour notice for all appointment cancellations so we may better serve all of our patients. Thank you!
